Welcome! As the new office manager at Elite Obstetrics and Gynecology it my pleasure to “officially” introduce myself to you. Unlike so many people who have relocated to the sunny south from elsewhere, I was actually born and raised in Miami, Fl as the youngest of six brothers and sisters. I moved to Broward County about 10 years ago, pursued a career in healthcare management, and have been happily managing doctor’s offices ever since. I truly enjoy working in the medical field and to increase my professional capabilities in this area I am currently completing a degree in Health Administration at Nova Southeastern University. Joining the team at Elite has been a blessing for me because not only do I love Dr. Sam’s vision for her practice, but as someone who is very family-oriented it’s great to work with such a wonderful group of women everyday. I look forward to doing my best to make sure that Elite Obstetrics and Gynecology continues to set the highest standard in women’s healthcare and overall wellness. I graduated from the Meriden-Wallingford School of nursing in 1975 after which I worked at the UCONN Medical Center as a registered nurse specializing in gynecology, infertility and high-risk obstetrics. Four years later I enrolled in a Women’s Health Care program at UCLA/Harbor General Hospital to become an Advanced Registered Nurse Practitioner. Upon successfully completing my national certification as an A.R.N.P., I continued to work at the UCONN Medical Center in their OB/GYN outpatient department. During this time I completed additional training at Harvard Medical School as well as Yale University, in the fields of colposcopy and pharmacology. I also opened and maintained an independent practice dealing with women’s health issues. Since relocating to Florida in 1993 I have worked for Planned Parenthood and other private medical offices focusing on obstetrics and gynecology. As well, over the past eight years I have complemented my nursing skills with aesthetic expertise via professional certification in the areas of laser hair removal, microdermabrasion, and the administration of cosmetic Botox. Now, as part of the Elite team , I’m thrilled that I can make great use of both my extensive background in gynecology and my love for aesthetic treatments!

Allow me to introduce myself, my name is Raquel Lopez and I am considered by those who know me to be a friendly, nice, outgoing and ambitious young woman. I graduated from Sheridan Technical Center in 2004 achieving my high school diploma. Most recently I attended Sanford Brown Institute from 2004 -2005 which gave me the opportunity to be trained as a Medical Assistant. Sanford Brown sent me to perform my internship with Dr. Sam, and I am proud to have subsequently joined her team upon completion of my program. As a sensitive and compassionate individual, I pride myself on giving our patients the attention and support I know they deserve. Thank you for your interest in me and our practice. Hello!! My name is Krystal Eubanks. I am considered a very loving, kind, easy going, and passionate person. I grew up in Deerfield Beach, Florida and attended Ultrasound Diagnostic School in 2002 from which I graduated as a Medical Assistant in 2003. I always believed I was put in this world to help and care for others which is what I aspire to do in my position as medical assistant. I am happy to have joined Dr. Sam in her new practice since I had the opportunity to work with her in the past. Hello, my name is Jayde Lamothe. I was born in Ft. Lauderdale and lived in Louisiana for about eight years before returning to Broward County a few years ago. I have two sisters, one brother and I love being an aunt to my niece and two nephews. In my spare time I enjoy cooking, skating, dancing, going to the beach, wakeboarding, fishing, and playing football. I am an avid U.M. fan and have loyalties to L.S.U. because of my Cajun father. I graduated from Sanford Brown Institute in 2005 receiving my license in Medical Assisting. Since then I have gained experience with geriatrics, proctology, and now I feel I have found my niche working with Dr. Sam in Obstetrics and Gynecology. Gabriela Vazquez is a licensed massage therapist, clinical aesthetician, and instructor with over 12 years of industrial experience in skincare, spa therapies, nutrition, and fitness. In addition to her qualifications in skincare, Gabriela has a bachelor’s degree in Business Studies and an Associate’s Degree in Business Administration. Gabriela was originally born in Munich Germany and is fluent in 5 languages including German, Spanish, Italian, and Czech. Her treatments include up-to-date hands-on techniques for the face and body utilizing Light Therapy (IPL/LLL), Oxygen Therapy, Ultrasound, and a range of alternative treatments from Customized Face Treatments, Therapeutic/Deep Tissue Massage, Scalp Stress Release, CranioSacral Therapy, Reflexology, and Acupressure to Reiki Energy Rebalancing.
