ELITE PLUS APPOINTMENTS
For busy women who WANT or NEED the convenience of “no wait”, VIP appointments with Dr. Sam within a guaranteed 24-48 hours of booking and with the option of using a private entrance, Elite is now offering Elite Plus appointments for a fee of $150.00.  Save time & enjoy V.I.P. peace of mind!

PLEASE NOTE:
The patient’s regular insurance will still be used to cover the usual expenses of this Elite Plus office visit.

HOURS OF OPERATION:
• Monday - Wednesday and Friday: 9:00 am - 5:00 pm
• Thursday: 11:00 am - 6:00 pm
• (off hours appointments available for our ELITE PLUS patients).

PLEASE NOTE: aesthetics and/or massage services can be added before or after any appointment.

OFFICE GYNECOLOGY SERVICES include but are not limited to:
Well woman visits, preconceptual counseling, surgical consults, STD screening, adolescent care and Gardasil HPV vaccinations, birth control consultations, menopausal management, treatment of fibroids and/or abnormal bleeding, pelvic pain, endometriosis management, ovarian cysts, etc.

OFFICE PROCEDURES: Essure permanent birth control, Thermachoice ablation therapy, colposcopy, cryotherapy, endometrial biopsy, hysteroscopy, treatment of genital warts, vulvar/vaginal biopsy, etc.

We ARE currently accepting new patients into the practice with the following appointment options:

ESTABLISHED AND NEW PATIENT NON-URGENT APPOINTMENTS are scheduled based on first availability with an active waiting list for last minute cancellation openings. New obstetrical patient interviews will be scheduled preferentially first availability.

OBSTETRICAL PATIENT PROBLEMS are discussed with Dr. Sam who determines if they need to be seen in the office the same day or directed to the hospital for more extensive evaluation.

ESTABLISHED GYNECOLOGIC PATIENT EMERGENCY issues are discussed with Dr. Sam and scheduled as per medically necessary as soon as possible.
